1. Fill-out a Pathmaker visit request form and fax it to:

  Visit Request Form A team of Pathmakers is identified based on availability. A Pathmaker will contact you to confirm the details of the presentations.
2. You will be asked to complete an evaluation form on the day of the presentation, which should be returned to your school board office as soon as possible.
